The LAFP is honored to recognize Louisiana Healthcare Connections as a 2023 Partner in Family Medicine!
Louisiana Healthcare Connections was established in 2012 to transform the health of communities, one person at a time, by delivering better health outcomes at lower costs. They have since grown into one of the largest of the state’s Healthy Louisiana Medicaid plans, with more than 450,000 members and a network of over 27,000 healthcare providers.
In 2018, LHC formally launched their Medicare Advantage product, Allwell from Louisiana Healthcare Connections, in 16 parishes across southeast Louisiana: Acadia, Ascension, East Baton Rouge, Iberville, Jefferson, Lafayette, Livingston, Orleans, Pointe Coupee, St. James, St. Landry, St. Martin, St. Tammany, Tangipahoa, Washington, and West Baton Rouge. To date, Allwell from Louisiana Healthcare Connections is providing 580+ members with access to nearly 6,000 healthcare providers.
LHC believes that community health must be locally driven and delivered in close collaboration with healthcare providers. From five offices in Baton Rouge, Lafayette, New Orleans and Covington, their 700+ Louisiana-based employees partner closely with physicians and community organizations to help all of their members achieve long-term improvements in their health.
By staying true to their mission and engaging members, providers, communities and employees, LHC is driving improvements in health while contributing to the economic growth of the state. For Louisiana Healthcare Connections, it’s not an innovative strategy for success—it’s a common sense solution for population health in Louisiana
