Nominations for the AAFP Public Health Award are open through March 1. The award honors champions who work outside their normal duties in promoting public health; who show strong leadership, innovation and advocacy skills; and whose work has aided both patient and community health. Members are welcome to nominate themselves or another family physician.

- A nomination form (self-nominations are welcome) that describes the nominee's contributions
- A separate document (limited to two pages) that chronicles the nominee's experience in public health leadership and provides clear examples of their work
- A letter of support from their chapter

Members and chapter executives will be notified that their nomination materials have been received; all nominations will be reviewed once the nomination process has closed.
The winner of the Public Health Award will be selected by the executive committee of the AAFP Commission on Health of the Public and Science, and will be officially announced at the AAFP Congress of Delegates meeting in Anaheim, Calif.
