Learning Objectives:

1. Review the various definitions of polypharmacy and multimorbidity and understand the extent of the problem.
2. Define the different criteria for deprescribing including BEERS, AGS and AAFP Choosing Wisely.
3. Medication Review and Implementing Sustainable Models to address Polypharmacy.
4. Identify and evaluate the opportunities and challenges in addressing polypharmacy in diverse patient populations.
5. Design and deliver small-scale educational interventions (STEP UP sessions).
