Transforming Clinical Practice Initiative

The AAFP and ABFM have partnered to participate in the Transforming Clinical Practice Initiative (TCPi) as the PRIME Support and Alignment Network (PRIME SAN).

This network will:

  • Help recruit family physicians into existing practice transformation networks;
  • Develop a member interest group (MIG) for ongoing learning and collaborative problem solving;
  • Provide maintenance of certification to participating practices;
  • Offer a TCPi-focused continuing medical education sessions at Family Medicine Experience (FMX);
  • Offer ABFM's Practice Performance Coaching Guide to support practice transformation;
  • Use unique, established communication and engagement channels (publications, national meetings, state chapter sessions) to disseminate learning among practices;
  • Develop Maintenance of Certification (MOC) modules to translate practice transformation successes; and
  • Offer the PRIME Registry(www.theabfm.org) to the first 6,000 ABFM board-certified family physicians to support quality measure tracking and reduce practice burden for Meaningful Use and PQRS reporting, MIPS (2017), and APMs to come.

How can your practice benefit from TCPi?

There are ten SANs and 29 practice transformation networks (PTNs) working with the 14 Quality Improvement Organizations (QIOs) to provide coaching, resources, and tools to help practices prepare for value-based payment models.

Each PTN is offering different resources and has identified a specific focus of engagement such as implementing processes for chronic care management to overall patient-centered medical home transformation.

Examples of PTN offerings include:

  • Communities of learning through in person collaboratives
  • On-site practice transformation coaches
  • Informatics expertise to expand data collection and analysis
  • Online educational content
  • Leadership and professional development

The Louisiana Academy of Family Physicians (LAFP) and the LHIT Resource Center (LHIT) worked together to develop a comprehensive toolkit to help guide physicians through the process of adopting an electronic health record (EHR) and achieving Meaningful Use.  The E-Louisiana HIT Toolkit is available to all members of the LAFP and LHIT courtesy of a grant awarded by the Physicians Foundation.

Get help with your EHR with the Louisiana REC

To help physicians implement health information technology, federal legislation created HIT Regional Extension Centers. There is one in Louisiana. The REC supports physician practices at any stage of EHR implementation through on-site consulting. Whether starting with a paper-based system or looking to optimize your current EHR to qualify for the federal incentives, the Louisiana REC can help you fully realize the benefits of electronic health records in improving efficiencies and patient care.

Value Based Payment Modifier

What is the Value-based Payment Modifier?

The value-based modifier program is a budget neutral payment modifier based on relative quality and cost of care. Quality and Resource Use Reports will provide the quality-related feedback. Medicare plans to adjust physician payments using quality outcomes data from the Physician Quality Reporting System (PQRS) and cost data from Medicare claims for fee-for-service patients.

According to the Patient Protection and Affordable Care Act, the modifier must be applied to all physicians and physician groups by January 1, 2017. This is a pay for value (i.e., quality relative to cost) program– higher value gets higher pay; lower value gets lower pay, based on quality tiering.

In 2019 and beyond, the value-based payment modifier will be replaced by the merit-based incentive payment system (MIPS).

By participating in these projects, you will have access to an online community of patients, caregivers, health care professionals, health systems, nonprofit organizations, federal agencies, private-sector partners and various community stakeholders to spread innovative and best practices, increase patient engagement, and structure processes for sustaining positive change. Below are highlights of the physician office projects:

Improve Cardiac Health, Prevent Stroke and Reduce Cardiac Disparities

  • Submit Physician Quality Reporting System (PQRS) cardiac data using a certified EHR for the Million Hearts™ ABCS: Aspirin Therapy for those who need it, Blood Pressure Control, Cholesterol Management, Smoking Cessation
  • Receive and review cardiac reports comparing individual data with peers in your state and implement workflow changes to improve your rates
  • Participate in virtual educational sessions and utilize online resources and forums

Provide Better Care at Lower Cost

Receive assistance with federal reporting programs and guidelines:
  • PQRS support
  • Quality Resource Use Reports (QRUR) interpretation
  • Value-Based Modifier Payment

Coordinate Prevention through Health Information Technology
