Transforming Clinical Practice Initiative
The AAFP and ABFM have partnered to participate in the Transforming Clinical Practice Initiative (TCPi) as the PRIME Support and Alignment Network (PRIME SAN).
This network will:
- Help recruit family physicians into existing practice transformation networks;
- Develop a member interest group (MIG) for ongoing learning and collaborative problem solving;
- Provide maintenance of certification to participating practices;
- Offer a TCPi-focused continuing medical education sessions at Family Medicine Experience (FMX);
- Offer ABFM's Practice Performance Coaching Guide to support practice transformation;
- Use unique, established communication and engagement channels (publications, national meetings, state chapter sessions) to disseminate learning among practices;
- Develop Maintenance of Certification (MOC) modules to translate practice transformation successes; and
- Offer the PRIME Registry(www.theabfm.org) to the first 6,000 ABFM board-certified family physicians to support quality measure tracking and reduce practice burden for Meaningful Use and PQRS reporting, MIPS (2017), and APMs to come.
How can your practice benefit from TCPi?
There are ten SANs and 29 practice transformation networks (PTNs) working with the 14 Quality Improvement Organizations (QIOs) to provide coaching, resources, and tools to help practices prepare for value-based payment models.
Each PTN is offering different resources and has identified a specific focus of engagement such as implementing processes for chronic care management to overall patient-centered medical home transformation.
Examples of PTN offerings include:
- Communities of learning through in person collaboratives
- On-site practice transformation coaches
- Informatics expertise to expand data collection and analysis
- Online educational content
- Leadership and professional development

Who cannot participate in TCPi?
Practices and/or family physicians participting in the following are not eligible to receive technical assistance from the TCPi program:
- Medicare Shared Savings Program
- Pioneer ACO Program
- Next Generation ACO Model
- Multi-Payer Advanced Primary Care Practice
- Comprehensive Primary Care Initiative
- Comprehensive Primary Care +
