The Robert Graham Center, in partnership with the Physicians Foundation, is recruiting Federally Qualified Health Centers (FQHCs) to participate in a study examining best practices for screening and follow-up on social determinants of health (SDOH) in primary care.

Participating practices will be asked to identify three staff members (e.g., a physician, nurse, and UDS/reporting lead) for one-hour interviews. Each participant will receive a $125 e-gift card, and the practice will receive a $1,000 e-gift card to support coordination.

If you successfully participate, please let the AAFP know that you were referred by the Ohio chapter so it can send our chapter a referral stipend.
