Step Two: Assembly Exhibit Booth

Exhibitor fee includes 8'x8' booth with (1) 2'x6' skirted table and 2 folding chairs, identification sign, pipe and drapes, registration list, listing in the program guide and meeting app, 100 word company/product profile in the meeting app with a link to your company's website. Electrical and shipping needs are at the exhibitor's responsibility. Important dates: cancellation deadline May 3, early bird deadline June 3, deadline for logo and program artwork June 3, booth assignments July 3. Step Four: Advertising Opportunities

The LAFP Conference Program is distributed to all individuals registered for the conference. It is not only an indispensable part of every attendee’s on-site conference materials, but also a historical document that many attendees take home and refer to in the days, months, and even years after the conference. The Conference Program includes meeting timetables, exhibitor descriptions and other essential information.

Cancellation & Payment Policy

CANCELLATION POLICY: A written notice of cancellation must be sent to the LAFP office at 919 Tara Blvd., Baton Rouge, LA 70806. Notices received on/before May 3, 2017 will receive a 50% refund. No refund or cancellation will be made after May 3, 2017.
PAYMENT POLICY: By submitting this online registration form, I hearby apply for exhibit space, sponsorship and/or advertising at the 70th Annual Assembly & Exhibition of the Louisiana Academy of Family Physicians (LAFP). I understand that when received by the LAFP, this application becomes a binding contract. By submitting the attached, I acknowledge that I have read and fully understand the 2017 Exhibition Prospectus, and that sponsorship can only be secured by submitting this signed, completed form.
