  • Cancellations must be received, in writing, no less than 75 days prior to first day of meeting
  • The Editor reserve the right to refuse any advertising or adjust publication size as necessary

I understand that when received by the LAFP, this application becomes a binding contract. By signing below, I acknowledge that I have read and fully understand 2016 Exhibit Prospectus, and that sponsorship can only by secured by submitting this signed, completed form.
NOTE: Full payment should be enclosed to ensure booth assignment.

CANCELLATION POLICY:
A written notice of cancellation must be sent to the LAFP Office, 919 Tara Boulevard, Baton Rouge, LA, 70806. Notices received on or before May 5, 2016, will receive a 50 percent refund. No refund or cancellation will be made on or after May 5, 2016
